Does your child wear headphones or earbuds?

We are all quite used to seeing others walking, jogging or working with earbuds or headphones.  Apparently, this use of audio equipment is becoming more widespread among younger children as well.

Why is this a concern?  Susan Woolford, a paediatrician involved in a National Poll on Children’s Health run by the University of Michigan says that “Over recent years we’ve mostly been concerned about teens overusing audio devices. But earbuds have become increasingly popular and prevalent among younger kids, exposing them to more intense noise on a regular basis.”

“Noise exposure risks to young children have historically involved loud singular events like concerts or fireworks, but parents may underestimate the potential harm from excessive use of listening devices. It may be difficult to know whether their child’s exposure to noise is healthy.”

Children are most likely to use these devices at home, school and in the car, report findings show. Half of the parents taking part in the poll agreed that headphones or earbuds help keep their child entertained.

It is worth keeping in mind the warnings about the dangers of prolonged or extreme exposure to loud noise to hearing loss or tinnitus.  Woolford reminds us that young children are more vulnerable to potential harm from noise because of their immature auditory systems.  Their ear canals are also smaller than adults, intensifying the levels of sound.  In addition, “Tiny hair cells inside the inner ear pick up sound waves to help you hear. When these get damaged or die, hearing loss is irreversible.”

Noise exposure among children can also affect their sleep, academic learning, language, stress levels and even blood pressure, she adds.

Woolford offers four tips to reduce risks of noise exposure to children through headphones and earbuds:

Monitor volume levels

Listen to the audio device used by your child and adjust the volume, Woolford says.

She recommends parents follow the 60/60 rule—children should be limited to no more than 60 minutes of audio devices a day at no more than 60% of the maximum volume.

“A good way to tell if an audio device is too loud is if a child wearing headphones can’t hear you when you’re an arm’s length away,” she said.

Use noise cancelling or volume limiting headphones

Consider the risk of noise exposure when you buy audio devices for your child by checking the information on device packages to identify products that limit the volume.

But some products marketed as “kid safe,” Woolford warns, do not limit the volume to 70 decibels.

However, children should avoid using noise-cancelling listening devices in situations when perception of sounds is crucial for safety.  These should not be used when a child needs to hear what is going on in their environment, such as walking or bike riding on public roads.

Make sure your child take breaks from listening devices

Children should have a daily “device-free” time, Woolford says. This may involve putting away or locking the child’s audio devices when time limits are up.

They may also encourage kids to enjoy things like music on a low volume in their rooms instead of using earbuds to reduce noise intensity.

Personal audio devices should also be avoided when children are sleeping or at bedtime, Woolford says.

Watch out for early signs of hearing loss

“Early signs of hearing loss may include asking for repetition, hearing ringing noises often, speaking loudly to people nearby, delayed speech, or lack of reaction to loud noises,” Woolford said.

We know that music concerts and other sources of loud sounds can lower auditory acuity in people both young and old.  But have you considered that children’s toys might put them at risk for hearing loss?

Katie Dease, of the Mayo Clinic writes that some toys can produce levels of sound equal to 90 decibels, with is as loud as or louder than a lawn mower.   Because children often play with toys at a close distance, holding them near their faces and ears, that volume can go up to as high as 120 decibels, which is as loud as an aircraft taking off.  At these levels, hearing protection is actually needed.

Do a sound test before buying

As a parents or other gift giver, it would be wise to  check the noise levels of a toy before buying.  Test the volume by holding the toy close to your face or crouch down to test how noisy it is for a child who plays on the ground.   If it sounds too loud to you, it’s too loud for the child.

Try to lower the sound level

Toys: Try placing clear tape over the speaker. This has been shown to lower the sound level and may be safer for children’s ears.  Check if the sound volume can be reduced or switched off certain toys.

Music and headphones:  If children and teens have a music player with earphones, parents should stand near their child when they are listening. If you can hear the music, it’s too loud.

Output-limiting headphones are an excellent gift option. In most cases, they automatically limit the output level to about 85 decibels, which is a safe volume for up to eight hours. Many devices, such as cellphones and tablets, have noise output-limiting settings that can be activated.

You also can invest in custom earbuds or noise-canceling headphones that help block background noise, allowing you to decrease the volume.

Instruments:

Some musical gifts are welcome but be aware of those that can do harm to hearing. For all musicians, but especially drummers, smart hearing health care is essential.

Older musicians may benefit from musician earplugs that limit sound without compromising musical integrity.

  • Drummers should try practicing with drum pads as much as possible. This will eliminate loud noiseswhile still offering a surface for rehearsing stick strokes.
  • Another approach is to limit practice to intervals. Try practicing, with ear protection, for 30 to 60 minutes, taking a break for the same amount of time to give your ears a rest, then going back to practice.

Over 1 billion teenagers and young people use headphones and earbuds and delight in attending venues where there is very loud music.  These practices put them potentially at risk of hearing loss, according to an analysis of available evidence, published in the British Medical Journal Global Health.

The World Health Organization (WHO) estimates that over 430 million people worldwide currently have disabling hearing loss. Young people are particularly vulnerable because of their use of listening devices, such as Smartphones, headphones and earbuds, and attendance at loud music concerts.  This is happening without adequate regulations to enforce safer practices.

Previously published research reports that headphones and earbuds are often played at volumes as high as 105 dB while average sound levels at entertainment venues range from 104 to 112 dB, exceeding permissible levels (80 dB for adults; 75 dB for children) even if for very short periods of time.

This current study aimed to estimate the prevalence of unsafe listening practices among teens and young adults to get a global estimate of the numbers who could be at risk of hearing loss, with the aim of informing evidence-based policy to safeguard hearing.

They obtained information from relevant studies published in English, French, Spanish and Russian, involving 12-34 year-olds, and reporting on objectively measured device output levels and length of exposure.

A group of 33 studies, corresponding to data from 35 records and 19,046 participants, was included; 17 records focused on use of listening devices and 18 focused on loud entertainment venues.

The researchers also estimated the global number of people who could be at risk of hearing loss by considering the estimated global population of 12-34 year-olds in 2022 (2.8 billion) and the best estimates of exposure to unsafe listening practices from listening devices or loud entertainment venues derived from the systematic review.

The pooled data analysis indicates that the prevalence of unsafe listening practices from both sources of loud sound is common worldwide—24% (for listening devices) and 48% (for attending loud music venues) among teens and young people.

Based on these figures, the researchers estimate that the global number of teens and young adults who could potentially be at risk of hearing loss as a result ranges from 0.67 to 1.35 billion.

The researchers acknowledge some limitations to their findings, including the varied study design—a particular feature of the studies on entertainment venues—and the absence of standardized methodology.

In addition, their estimates did not account for some potentially influential factors, such as demographic details and recent changes to policy on safe listening in some countries/regions.

Nevertheless, their findings prompt them to conclude, “There is an urgent need for governments, industry, and civil society to prioritize global hearing loss prevention by promoting safe listening practices.”
